Parents-> Getting Ready For Camp-> Health Form and Medical Information
Please be certain that you have filled out the online Health History form, and either uploaded or sent the faxback Physician's Exam, Immunization Form, and SIGNED Parent Authorization for treatment, by May 1 (first term) and June 1 (second term).
Doctors and hospitals will not treat children without a parent’s signature. Current information about medicine being taken and health concerns should be fully detailed in the online Health History form.
Regulations require that all prescription and over-the-counter medications be accompanied by a written authorization from a licensed health care provider.
Both prescription and over-the counter medications must be in original containers and labeled with camper’s name and dosage. At the end of the term, parents may pick up their child’s remaining medications at the health center. For campers who return home by air, medications will be mailed home.
Medical CareWe have modern well-equipped health centers with registered nurses on duty in each camp, and we work closely with physicians in Woodland Park and Colorado Springs. It is a 30-minute drive to Pikes Peak Regional Hospital. Parents will be notified by phone / email if your child requires offsite care for illness or injury.
InsuranceMedical expenses incurred by your child during camp will be covered by your family health insurance. Please include and/or update current insurance information on the online Health History form.
Families who live out of Colorado or whose insurance requires pre-authorization for use within a network of providers will want to determine how to access your insurance benefits if your child should need emergency or sick care while at camp.
Camp will pay required co-pays at the time of service; these will then be billed as a part of the camper’s expense account. We also have a Physician’s Assistant who lives on the camp property and is able to diagnose and treat common ailments promptly so that campers miss less program time. Her charge is $40 and is not refundable through insurance. The charge will be added to the camper’s expense account.
Campers who have a more serious or difficult to diagnose illness/injury will be transported to one of the camp’s doctors in Woodland Park or Colorado Springs.
